I am 18 years of age or older, or I am 16–17 years of age and have a parent or legal guardian present with valid photo ID for both myself and my guardian, in compliance with Florida Statute and DH 4146. 2. I am not under the influence of alcohol or drugs. 3. I do not have any medical conditions, skin conditions, allergies, or blood disorders that would prevent me from safely receiving a tattoo or piercing, or I have disclosed all such conditions to my artist prior to the procedure. 4. I understand that tattooing and piercing are permanent body modifications. Results vary based on skin type, aftercare, and individual healing. 5. I have reviewed and understand the aftercare instructions provided by Tattoos Near Me and agree to follow them to promote proper healing. 6. I release Tattoos Near Me, its owners, artists, and staff from any liability for infection, allergic reaction, or other complications arising from failure to follow aftercare instructions. 7.
